Your rights under HIPAA
Federal law (the Health Insurance Portability and Accountability Act, or HIPAA) gives you these rights over your health information:
- Ask to see or get a copy of your health records
- Ask us to correct your health records if you think they’re wrong or incomplete
- Ask us to contact you in a specific way, such as at a different phone number or address
- Ask us to limit what we use or share
- Get a list of those with whom we’ve shared your information
- Get a copy of our privacy notice
- Choose someone to act for you, such as a legal guardian
- File a complaint if you feel your rights have been violated
Extra protection for substance use treatment records
Records of substance use treatment have additional protection under federal law (42 CFR Part 2). In general, they can’t be shared without your written consent, except in limited situations the law allows.
How to file a complaint

You can also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, online at ocrportal.hhs.gov, or by calling (800) 368-1019 (TDD (800) 537-7697).
You will not be penalized for filing a complaint.
